​COVID QUESTIONNAIRE

The district will require everyone (campers/coaches/counselors) to complete a COVID-19 Health Screening Questionnaire -

 

Please print out and fill out the ​COVID QUESTIONNAIRE before entering clinic or camp each day .  It is also found on the Athletic Webpage.

NO SCREENING FORM, NO ENTRY

Monday's in the Fitness Room at Baker:

​

Face Coverings:

Face Coverings are still required while being inside the building to be used as tolerated. Mask breaks are encouraged as long as athletes can socially distance themselves from each other

 

Entering & Exiting Building at Baker H.S.:

Exit 2 will be utilized which it the far exit by the small gym.

Tuesday's with Coach & Summer Camp:

Face coverings:

Face coverings are not required

 

Tuesdays with Coach will meet at field 6 (outfield of baseball field) each night to either practice on the grass or go down to the turf.

Summer Camp will be held on the turf.
